What is EndoSim?
Your Mission
Each case presents a real patient with a dental complaint. Your job is to act like a clinician — gather information, run tests, and make a diagnosis.
Two Diagnoses Required
Every case needs two diagnoses per AAE 2013:
1. Pulpal diagnosis — what is happening inside the tooth?
2. Apical (periapical) diagnosis — what is happening around the root tip?
You also select the correct treatment and rate your confidence.
💡 This is not a quiz — it is a simulation. Take your time, think clinically, and use the investigations like a real clinician would.

Step 1 — Gather Information
Tap tools to reveal findings
After reading the patient's complaint, tap the investigation buttons on screen to reveal clinical findings. Each button you tap unlocks a test result — just like examining a real patient.
💡 You do not need all tools. Using 4 or fewer earns +2 efficiency points. Using more than 7 costs −1 point.

Step 2 — Make Your Diagnosis
When you are ready
Tap Diagnose at the bottom of the screen. A panel opens where you select:

Pulpal Diagnosis — Choose from 7 AAE 2013 categories (Normal Pulp, Reversible Pulpitis, SIP, AIP, Necrosis, Previously Treated, Previously Initiated).

Apical Diagnosis — Choose from 6 AAE 2013 categories (Normal, SAP, AAP, Acute Abscess, Chronic Abscess, Condensing Osteitis).

Treatment — Select the correct ESE 2023 treatment pathway.

Confidence Rating — How confident are you? 1 = not sure, 5 = very confident. Helps calibrate your self-awareness.
💡 There is a 3-minute timer per case. If time runs out, the diagnosis panel opens automatically.

How Scoring Works
Maximum score: 17 points per case
+5 Pulpal correct
+5 Apical correct
+2 Treatment correct
+2 History first
+2 Efficiency ≤4 tools
+1 Thermal before Percussion
−1 Hint used
−1 Over-investigation >7 tools
After each case
You receive immediate feedback showing:
• Whether each diagnosis was correct
• The clinical rationale referenced to AAE 2013 and ESE 2023
• Which investigation tools you missed
• The ESE hierarchy note (thermal vs percussion order)
• Clinical significance of spontaneous pain in this case
⚠️ Pass mark is 50% — you need at least 8.5 / 17 to pass a case.
